    Before any PMU procedure, each client must complete the required consent and information form. This helps provide a safer, clearer and more professional experience.

      Consent for Brow Micropigmentation

      I give my consent for brow micropigmentation.I understand that results may vary and that a touch-up may be necessary.I confirm that I have received the aftercare instructions.

      Consent for Lip Blush

      I give my consent for lip micropigmentation, also known as Lip Blush.I understand the risk of cold sores and that I may need antiviral medication.

      Deposit and Cancellation Policy

      • A non-refundable $50 deposit is required.

      • A 24-hour rescheduling window is required.

      • No-show appointments result in loss of deposit.

      • There are no refunds under any circumstances.

      Pre-Care Instructions

      • No caffeine 24 hours before the appointment.

      • No alcohol 48 hours before the appointment.

      • No ibuprofen 48 hours before the appointment.

      • No facial procedures 2 weeks before the appointment.

      Aftercare Instructions

      • Keep the area clean for 24 hours.

      • No makeup for 7 days.

      • No sun or sweat for 10 days.

      • Use sunscreen after healing.
